We are seeking a proactive and community-focused individual to support public health education and outreach initiatives. This role involves planning and delivering presentations, organizing events, and connecting individuals with healthcare resources. The position emphasizes prevention efforts related to opioid misuse, tobacco use, and vaping, while promoting awareness of cessation programs, safe medication disposal, and smoke-free policies. This is a grant-funded position and is contingent upon continued funding approval.

Key Duties:

• Identify target populations based on health and social service needs
• Assess community needs using relevant data and trends
• Develop and distribute educational materials tailored to specific audiences
• Organize and facilitate workshops, outreach events, and presentations
• Deliver health education to diverse groups, including youth and families
• Promote awareness of local health resources and prevention programs
• Advocate for smoke-free policies and educate on secondhand smoke risks
• Support multi-disciplinary teams in coordinating care and services
• Provide feedback to healthcare providers to improve service accessibility
• Prepare and conduct surveys to support data collection for grant activities
• Maintain confidentiality and manage records in accordance with regulations
• Perform clerical tasks including data entry, phone support, and report preparation
• Adapt to evolving community needs and funding requirements

Qualifications:

• High school diploma or GED, or equivalent combination of education and experience
• Valid driver’s license
• Community Health Worker Certificate
• Knowledge of public health principles, community resources, and outreach strategies
• Strong communication skills, including public speaking and cultural sensitivity
• Ability to work independently and collaboratively with diverse populations
• Proficiency in organizing events, managing records, and performing clerical duties
• Experience working with multi-disciplinary teams and advocating for public health policies
• This position requires reference checks and a thorough background check
